PSYCHIATRIC ILLNESS DURING PREGNANCY

Colorado Postpartum Care and Perinatal Mental Health Utilization Analysis (2019-2023).
Executive summary of 2019–2023 Colorado data shows 1 in 3 births involved a parent with a diagnosed mental health condition, and 2 in 3 postpartum individuals with a mental health diagnosis received no mental health services during or after pregnancy, despite improvements in screening and follow-up.

2025 State Report Cards.
The Policy Center for Maternal Mental Health’s 2025 report cards grade states across providers/programs, screening and reimbursement, and insurance/treatment; the overall U.S. grade improved from D+ to C-, with 26 state grades improving and 19 states now earning Ds or Fs.

Gender-Affirming Perinatal Psychiatry for Transgender and Gender Diverse People.
Kamceva M, Pendse RS, Keuroghlian AS. J Clin Psychiatry. 2025 Nov 10;87(1):25com15907.
Review article outlining principles and gaps in gender-affirming perinatal psychiatric care for transgender and gender diverse people.

Conceptual framework on barriers and facilitators to implementing perinatal mental health care and treatment for women: the MATRIx evidence synthesis.
This evidence synthesis provides a framework describing barriers and facilitators to perinatal mental health screening and treatment implementation, including persistently low depression screening rates in many settings.

MEDICATIONS AND PREGNANCY

Association between gestational hypnotic benzodiazepine receptor agonists exposure and adverse pregnancy outcomes: a systematic review and meta-analysis.
HBRA exposure during pregnancy may lead to a small but significant increase in the risk of PTB, SGA, and LBW. HBRA is not associated with an increased risk of birth defects; limitations include residual confounding by psychiatric illness and co-medications.

POSTPARTUM PSYCHIATRIC ILLNESS

Breastfeeding interventions for preventing postpartum depression.
Lenells M, Uphoff E, Marshall D, Wilson E, Gustafsson A, Wells MB, Andersson E, Dennis CL. Cochrane Database Syst Rev. 2025 Feb 18;2(2):CD014833.
There is low-certainty evidence that psychosocial breastfeeding interventions may prevent postpartum depression in the short term and increase the duration of breastfeeding in the long-term.

Reduction of neonatal intensive care unit (NICU) parental perceptions of child vulnerability and risk of vulnerable child syndrome utilizing cognitive behavioral therapy: randomized controlled trial.
A parental trauma-informed CBT intervention in the NICU decreased parental perceptions of child vulnerability and lowered risk of developing Vulnerable Child Syndrome.

MEDICATIONS AND BREASTFEEDING

Psychotropic Medication Exposure via Breast Milk: A Population-Based Descriptive Study in Denmark.
Liu X, Madsen KB, Zhu JL, Munk-Olsen T, Damkier P, Lupattelli A, Zoega H, Nordeng H, Kjeldsen MZ, Mægbæk ML, Madsen MG, Bergink V, Bliddal M. Paediatr Perinat Epidemiol. 2025 Sep;39(7):612-625.
Approximately 2% of exclusively breastfed infants in Denmark were exposed to psychotropic medications via breast milk, most commonly antidepressants (15.0 per 1000, primarily sertraline), followed by hypnotics/sedatives and antipsychotics.

Cognitive Outcomes of Children Exposed to Selective Serotonin Reuptake Inhibitors Through Breast Milk.
In a cohort of children prenatally exposed to SSRIs, additional exposure via breastfeeding was not associated with reduced IQ scores, suggesting breastfeeding can be encouraged during SSRI treatment.

OTHER TOPICS IN WOMEN’S MENTAL HEALTH

Systematic Review and Meta-Analysis: The Associations of Prenatal Exposure to SARS-CoV-2 Infection and COVID-19 Vaccination With Child Neurodevelopment.
Rizzo M, Tubassum R, Kaplan CA, Konde M, Martin L, Gigase F, de Witte L, Bergink V, Rommel AS. J Am Acad Child Adolesc Psychiatry. 2025 Oct 27:S0890-8567(25)02151-3.
Prenatal SARS-CoV-2 exposure was associated with early impairments in hearing, fine motor, and problem-solving skills, which appear to resolve with time. No associations were identified with atypical neurological or neuroimaging outcomes. No adverse neurodevelopmental effects were reported in the two studies that examined prenatal COVID-19 vaccination.pubmed.ncbi.nlm.nih?

Maternal infection – but not inflammation – is associated with attention-deficit/hyperactivity disorder symptoms during childhood: a population-based cohort study.
Zarchev M, Gigase FAJ, de Witte L, Cecil CAM, Hillegers MHJ, Ospina LH, Suleri A, Rommel AS, Muetzel RL, Bergink V. Brain Behav Immun. 2026 Jan;131:106134.
The findings suggest that maternal fever and infections during pregnancy are linked to offspring ADHD symptomatology, with effect sizes smaller than other perinatal ADHD risk factors; markers of chronic inflammation (cytokines, CRP) were not associated with ADHD.

Maternal Use of Acetaminophen (Paracetamol) During Pregnancy and Neurodevelopmental Disorders in Offspring: A Reasoned Evaluation of Risk.
Andrade C. J Clin Psychiatry. 2025 Nov 10;86(4):25f16187.
There are many unmeasured confounds that, even if weak, could negate the relationship between acetaminophen and neurodevelopmental disorders (NDDs). Sibling analyses suggest that shared genetic and shared environment risk factors (rather than acetaminophen exposure) may explain the NDD risk.

The pregnancy puzzle facing women stopping GLP-1s.
Women with obesity who stopped taking GLP-1 receptor agonists before or early in pregnancy gained more weight than those who had never used these medications, based on an observational study published in JAMA.jamanetwork?

Thyroid Imbalance During Pregnancy Tied to Heightened Autism Risk in Offspring.
A cohort study in the Journal of Clinical Endocrinology & Metabolism found that persistent thyroid hormone imbalance across pregnancy was associated with increased ASD risk in offspring.
