This week’s roundup highlights three especially compelling themes: emerging neuromodulatory strategies for PPD (brexanolone, ketamine assisted psychotherapy, and prophylactic esketamine), the rapid expansion of AI tools for prediction and screening of PMADs, and the mental health fallout of post-Dobbs abortion policies. Together, these studies raise timely questions about how we will balance innovation with equity: who gets access to novel treatments, what data will be used to power AI tools, and which communities bear the greatest burden when reproductive rights are curtailed.

Trajectories of perinatal sleep quality from early pregnancy to six weeks postpartum and their risk factors: A multicenter longitudinal study in China

Researchers identified three sleep quality trajectories during the perinatal period: “stable-good” (n=697, 57.6%)”, “stable-poor”(n=495, 40.9%), and “severe-improving” (n=18, 1.5%). Early-pregnancy stress, anxiety, and pre-existing sleep problems are significant risk factors for poor sleep. These findings highlight the need for early screening and intervention for sleep and mood issues in prenatal care, starting from the first trimester.

Risk factors for treatment resistance among women with postpartum depression in a nationwide study

Among the 58,618 patients with PPD, 3,522 (6.0%) met the criteria for treatment-resistant PPD (TRPPD) during 1?year after PPD diagnosis. Lower educational level, lower household income, being non-cohabiting, smoking in early pregnancy, delivery by cesarean section, pre-existing physical conditions and pre-existing psychiatric disorders were significantly associated with a higher risk of TRPPD.

In this cohort study, state-level abortion bans following Dobbs were associated with a disproportionate increase in the risk of PPD among women and adolescents in low-SES communities. These findings underscore the need for targeted mental health support and policy interventions to mitigate the unequal burden of such legislation on vulnerable populations.
Brexanolone infusion produces sustained anti-inflammatory and neurotrophic effects in patients with postpartum depression that predict symptom improvement

Because the study was small, the findings should be interpreted as a pilot study, but they indicate that brexanolone promotes both rapid and sustained anti-inflammatory and neurotrophic effects supporting lasting symptom remission in PPD.
Can Social Work Interventions Improve Perinatal Mental Health Outcomes? A Systematic Review

A total of 32 studies were included and described with narrative synthesis. Social workers were embedded in various settings including obstetrics and gynecology offices (40%), other outpatient clinics like primary care (13%), hospitals (22%), and community settings like home visitation programs (25%). More than half of studies (n = 20) assessed the efficacy of a social work perinatal mental health intervention at reducing depression, anxiety, or another mental health outcome. Of these studies, almost all (n = 18) reported at least one significant improvement in a mental health outcome.
Artificial intelligence in the prevention and early detection of postpartum depression: a systematic review and meta-analysis

16 studies met inclusion criteria. Machine learning models showed greater accuracy than traditional methods. Integration of AI with medical records and social media data enabled earlier, personalized detection. Reported challenges included algorithmic bias, data privacy, and implementation barriers. Pooled sensitivity was 69% (95% CI: 55-81%; n=277,496) and accuracy 79% (95% CI: 73-85%; n=306,156).

Using AI-powered mobile applications for mental health screening can help reduce maternal mental health disparities among Black mothers who are pregnant or parenting in the United States. A maternal health education question and answer mobile application chatbot has the potential to intervene in the maternal depression cascade, specifically screening. Findings indicate that the mobile application demonstrated promise in the application’s usability to screen for maternal health depression indicators.

The total incidence of PPD in the esketamine group (11.59%) was significantly less than the control group (20.89%) [adjusted relative ratio (RR), 0.57; 95% CI: 0.35 to 0.94; P = 0.028] as was the incidence at 7 days postpartum (4.89 vs. 15.19%; adjusted RR, 0.32; 95% CI: 0.15 to 0.72; P = 0.005). However, there were no significant differences in PPD incidence and EPDS scores at 1, 2 and 3 months postpartum, respectively.
Association between trajectories of sleep quality and postpartum depression: a group-based trajectory model and computer-simulated network analysis

Two distinct trajectories of sleep quality were identified: “increasingly poor” trajectory (41.4%), which exhibited a markedly higher rate of postpartum depression (OR = 2.75, P < 0.001), and “stably good” trajectory (58.6%).

The transition to parenthood involves significant changes, and while mobile apps offer promising perinatal support, their impact on couples’ psychosocial and parenting outcomes remains uncertain. To evaluate the impact of mobile apps on psychosocial and parenting outcomes during the perinatal period, a systematic review of randomized controlled trials (RCTs) was conducted. The findings indicate that perinatal mobile applications may have limited impact on psychosocial and parenting outcomes in the early postpartum period.

After Trump suggested, without sound evidence, that women who take Tylenol during pregnancy risk causing autism in their children, Steven Fleishman, president of the American College of Obstetricians and Gynecologists (ACOG), called such suggestions “highly unsettling”. After the Food and Drug Administration (FDA) convened a panel focused on the risks of taking antidepressants during pregnancy, Fleischman slammed it as “alarmingly unbalanced”. And after the Centers for Disease Control and Prevention (CDC) stopped recommending Covid vaccines for pregnant women, Fleischman accused its vaccine advisory committee of failing to follow “longstanding tradition of robust, unbiased review of reputable scientific evidence”.
